[Spambayes] Manually uninstall SpamBayes Outlook?

I have uninstalled SpamBayes using add/remove programs but it still show up in my Outlook toolbar and it's still filtering email; although, the toolbar add-in isn't working (i.e. the menu doesn't appear, I can't get to SpamBayes manager, the Spam and Not Spam buttons don't do anything, etc.)  How can I manually uninstall this add-in?
