Does 2.1.5 formally require plus addressing support in the MTA?
That's only required if you let the MTA generate the VERP.
Not quite. It minimally requires plus addressing to be enabled in the MTA (assuming it supports it).
I thought that was still optional if you didn't use any of the VERPish supports.
Right, modulo the plus address extension removal.
I'm not familiar with the various common MTAs current OTB configuration. When last I checked (year+ ago) most didn't enable plus addressing support by default.

Only if the MTA in question is receiving a VERPed recipient
address. Mailman will never generate that kind of address (although users could subscribe with addresses of that form, but this is a separate matter), and will handle the generation of the VERPed envelope sender address itself.
The only time the VERPed address comes into play is if there is a
bounce generated to the envelope sender address that is created by Mailman, and even then it's just a matter of having the MTA pass everything it gets to mailman.
You might want to check again. I haven't seen any such problems
on the Mailman lists I've been managing for a couple of years.

Nope. Not if you configure the MTA to use Berkeley DB for aliases, and you maintain the VERP aliases in the DB. It _is_ possible. It's more work than standard MTA supported VERP, but it is doable.
