Hi all, our production mailman suddenly stopped sending mail to moderator for required list approvals and subscription approvals. Ex: when someone scribe to mailing list we have configured to receive the mail to "moderator/administrator" for approval but in our case administrator/moderator not getting mails on their mail box. each time we need to logon to list and see for the pending approval or subscriptions which is not practical. We are sing "postfix." as mail agent for mailman. Below are the error messages. /var/log/mailman/smtp-failures

The above sets of log messages do not relate to the same email messages. The messages from /var/log/maillo(g) are posts being delivered (successfully) to the it-hyd@intra.siliconimage.com list. The messages from /var/log/mailman/smtp-failure(s) relate to the failure of the subsequent delivery of the owner notice to it-hyd-owner@intra.domain.com.
Have you munged the domain in the smtp-failure messages. These domains are normally the same. Do you have aliases in postfix for the it-hyd-owner address as well as the it-hyd address? A full set of aliases for this list is
it-hyd: "|/usr/lib/mailman/mail/mailman post it-hyd" it-hyd-admin: "|/usr/lib/mailman/mail/mailman admin it-hyd" it-hyd-bounces: "|/usr/lib/mailman/mail/mailman bounces it-hyd" it-hyd-confirm: "|/usr/lib/mailman/mail/mailman confirm it-hyd" it-hyd-join: "|/usr/lib/mailman/mail/mailman join it-hyd" it-hyd-leave: "|/usr/lib/mailman/mail/mailman leave it-hyd" it-hyd-owner: "|/usr/lib/mailman/mail/mailman owner it-hyd" it-hyd-request: "|/usr/lib/mailman/mail/mailman request it-hyd" it-hyd-subscribe: "|/usr/lib/mailman/mail/mailman subscribe it-hyd" it-hyd-unsubscribe: "|/usr/lib/mailman/mail/mailman unsubscribe it-hyd"

It looks to me like you have an *internal* network configuration issue, such that "intra.domain.com" doesn't recognize the postfix host (also "intra"??) as an authorized local mail originating host. So the message is treated as an unauthorized outsde-to-outside relay and refused.
It appears that you have redacted your host names, so it's hard to make further guesses about what the actual problem is. But it's not Mailman, I think. It's your MTAs or DNS.
