Is the list's subscribe_policy set to Confirm or Confirm and approve?

What response does the user get when the subscribe form is submitted?

Is there anything in Mailman's 'error', 'smtp' or 'smtp-failure' logs with timestamp of the time of form submission?

Is the subscription actually pending? Use Mailman's

bin/dumpdb lists/LISTNAME/pending.pck

or the script at <http://www.msapiro.net/scripts/list_pending> to see.
