I've just noticed there's another version of the form which asks for my bugs.python.org username - that's takluyver. I hope that's OK, but let me know if I should fill in the other form.
(If someone wants to clear up the discrepancy, the license FAQ links to http://www.python.org/psf/contrib-form.html , which doesn't ask for a username, while the 'Contributor forms' page links to http://www.python.org/psf/contrib/contrib-form/ , which does).
