The original authors of the BM3D paper have a Matlab implementation at
 http://www.cs.tut.fi/~foi/GCF-BM3D/

However, if I remember correctly it relies on some pre-compiled MEX files so isn't really open source. It is free to use for non-profit/academic purposes, though.

There was also a C++ implementation published in IPOL
