[AstroPy] Page No, and Chapter Heading Problems with PyFits Manual

I'm looking (Subject) at the v 1.0, July 2005, manual. The TOC chapter 
#s and page numbers do not agree with the body of the manual. For 
example, Chapter 2 is used twice in the body. It looks like page numbers 
start over in most chapters.
