[AstroPy] Astropy Performance Survey

Hi folks,

The focus of the next astropy release (3.1)
<https://github.com/astropy/astropy/projects/4>  is on improving the speed
and memory performance of the astropy core package. As a result, we are
soliciting input from users to help focus these efforts via a super short
survey (5 questions).
