Summary: patch to add a DEFAULT_UCFIRST_REAL_NAME option

Initial Comment:
This patch adds a DEFAULT_UCFIRST_REAL_NAME option,
defaulting to 1,
which allows site admin to request that new lists all
be created with
the list name being all lowercase by default, instead
of the mixed case
name.
