How did you install/upgrade-to 2.1.15?

It appears you have the 2.1.15 version of Mailman/Cgi/admin.py but some
older version of Mailman/htmlformat.py or somehow have some local mod
which overrides or replaces the __init__ method in the htmlformat.Form()
class.
