Number one is just about moderation.  Numbers 2 and 3 are about whose posts 
are allowed through.  If you are running a moderated list then (I believe) 
the people from #3 are also allowed straight through.  There is a chart for 
the 2-3 interaction in the admin docs that I have prepared at 
www.aurora.edu/~ckolar/mailman.  There is really no interaction between 1 
and 2 as all postings are held for approval (even if they come from an off 
list poster).
