Exim has a pleasing concept of a filter which runs on a message
immediately prior to external delivery (hand off to another MTA).
If you do it at that level, yes, it is expensive, but you can cut
the Mailman generation/protocol expense and instead amortise if over 
the mail queue delivery time.
