Properly, and this may or may not have anything to do with Hotmail
and how it does or variously does not implement thread support,
threading is done via the following headers:

  Message-ID:
  In-Reply-To:
  References:

Message-ID provides a unique identifier for a message.  In-Reply-To
lists the MessageID that the current message is a reply to.
References lists prior MessageIDs in the current thread.

My MUA (exmh) supports and properly sets all three.  Checking of the
headers of the other messages you identify may reveal that other
MUAs don't (which is fairly common).
