[Mailman-Users] Message has implicit destination

This is really annoying, and I don't know why it happens or how to 
fix it.  I have some user who are subscribed to list who whenever 
they try to post a message have to wait for approval :


As list administrator, your authorization is requested for the
following mailing list posting:

     List:    blah blah
     From:   blah blah
     Subject: blah blah
     Reason:  Message has implicit destination

At your convenience, visit:

     blah blah blah

to approve or deny the request.


What is an implicit destination anyway?  How do I fix this?
