[Mailman-Users] Subscribe confirmation message not send

Hi all,
Got an outstanding issue with Mailman 2.1.9 included with Apple
Leopard server 10.5.6: suddenly the subscribe confirmation messages
never send out anymore.
Here is a little cut & paste out of the error log:

Seems like everything is posted at the correct log but no confirmation
message is sent out. Any advice would be greatly appreciated.
