Could also be something as simple as two memberships- joeuser at domain.com and 
joeuser at mail.domain.com. I've had that happen to me, both as a user and as a 
list admin.

(One case of that- the mail host sent as "user at saltmine.radix.net" but 
saltmine didn't directly receive mail; incoming had to go to 
"user at radix.net". I had two sub's to the same list, one set as nomail.)
