Summary: Index generation for Documenting Python

Initial Comment:
I was looking for a definition of the \ttindex
macro today and couldn't find one.  My search
was made more difficult by the lack of an index
in Documenting Python.

The attached file tweaks doc.tex and ltxmarkup.sty
to generate an index.  I don't know if my mods to
the .sty file are correct.  I was just mimicing
what I saw in the cfuncdesc macro def'n in
python.sty.
