Summary: _csv.c leaks when dialect creation fails

Initial Comment:
test_csv currently leaks 48 refs per run, and they all
seem to be fixed by this simple patch.

(The problem is that when you create a Dialect
instance, but then validation fails for some reason,
you need to decref the partially created instance
before returning NULL).
