It surprised me that cpyext didn't use real C types everywhere at the interface,
anything less seems weird. Of course the wrapper can convert whatever it likes.

There doesn't seem to be any provision for typedefs at the rffi level (size_t is
simply declared as integer type) so if something is going to be cleaned up, this
should probably be included.
