Fixed in Doc/perl/l2hinit.perl revision 1.58.

Yes -- this is the same bug (I hope!).  The "Extending &
Embedding" manual's index is also missing.

The docs included with the Windows 2.2b2 distribution only 
have indexes (genindex.html) for the lib and mac 
subdirectories.  The indexes for the C API and Python 
Reference Manual are missing (I assume this is the same bug 
as reported here).
