Summary: mkhowto --iconserver doesn't do anything

Details: The --iconserver option to mkhowto doesn't seem to work.
The init file contains a $ICONSERVER='whatever' line, but LaTeX2HTML (the
correct version, 99.2b8) seems to just be ignoring the setting.

This is due to agressive caching of the <img> tags for the icons; fixed in
Doc/perl/l2hinit.perl revision 1.51.
