Log Message:

make_head_and_body():  Replace the stock definition of this so that
        there's not a comment before the <HTML> tag.  This caused a
        problem with MSIE 5 on NT4 with SP4, as reported by Ionel
        Simionescu <ionel@psy.uva.nl>.
