[Python-Dev] Py_PROTO elimination in Include/

  I've removed Py_PROTO from the headers in the Include/ directory,
with the exceptions of the my*.h headers.  I'll leave it for others to
fix those, merging with pyport.h as appropriate.
  I think I'll stop for a bite to eat.  ;)
