You are right; I was wrong.

This is indeed what the current algorithms will produce.
In http://optarbvalintenc.blogspot.nl/2015/04/the-new-proposal.html I propose to improve that as follows:

0: 00
1: 01
2: 10, 00
3: 10, 01
4: 11, 00
5: 11, 01
6: 10, 10, 00
etc.
