Is SpamBayes listed in Outlook's (what version are you using?) disabled
items list?  Help->About Microsoft Outlook->Disabled Items.

Is SpamBayes listed in the COM addins list?  Tools->Options->Other->Advanced
Options->COM Add-ins.
