The Outlook addin relies on Outlook to tell it when new messages have
arrived.  Unfortunately, this part of Outlook fails under load:

Whole the article there only mentions OL2000, ItemAdd isn't more reliable in
OL2002 or OL2003.  The article also suggests that it's limited to
interaction with Exchange Server (under "Cause"), but that's not so either;
as it says in "More Information", it doesn't really matter what the source
of "the number of items necessary to reproduce this problem varies depending
on the size of the items".
