Barracuda adds a prefix, as does the Exchange module in Trend Micro's
Office Scan. (Neither adds the prefix Jan that complained about,
however.)

The only way to know for sure is to disable or remove it and anything
else that might be adding the prefix. But I'd start by talking to the
mail server administrator in case they're running some spam filter
before the message reaches your machine.

I don't think that's coming from SpamBayes. Maybe Outlook or your mail
server adds that as a service of sorts; you could try contacting the
administrator of your mail server. See

unfortunately.)
