In my experience it generally means you have to programs trying to write to
the database at the same time.  Two copies of the Outlook plugin or two
copies of the POP3 proxy or one of them and any other SpamBayes application
which writes to the database.

Of course, if you saw a Blue Screen of Death that would be a good candidate
as well.
