"Create New Issue" form is very much discouraged (and may even go away
at some point of time).

I am not sure. Perhaps, a recommendation can be added to
http://www.python.org/dev/patches/ that mentions using Rietveld. In
addition, this link can also be shown right above the upload form in
Rietveld issue (may be a simple template change). A slight variation
would be to put the text "please consider uploading the patch at
http://codereview.appspot.com" above the upload form.

We can't be sure that all the members in the nosy list would want to
be automatically added to Rietveld issue (assuming that is what you
are implying). I guess there is no simple way of integrating Roundup
and Rietveld currently. I did bring this up on Rietveld list quite a
while back to which Guide replied that he didn't see any such need.
