I think I've now got scrolling to work on iOS/Safari
and Opera almost works... (a slight glitch when the
width is within a particular size range) If anyone
uses Opera mobile on a mobile device please let me
know if it looks OK.

I've also added a "menu button" that hides/shows
the TOC pane which should help on phones and other
narrow devices.

Once more any feedback is appreciated. Please
send direct to me rather than the list.

If this works I'll finally be able to start updating
the content!...
