Summary: xmlproc and external entities

Initial Comment:
Problem descriptions attached in form of two mails.

#1 is ignoring of entities in the internal subset 
after reading an external entity.

#2 is nesting of external entities, at depth 2 xmlproc 
does not accept xml decls.
