Thanks for responses thus far...I have tried to look into them, but I'm
having trouble understanding them.

RESPONSE#1:

Here is some more info...

When I send an e-mail to the Admin of any group
(<LISTNAME>-admin at mailman.ccsds.org) I get the following error in a
returned e-mail:

ERROR MESSAGE:
This message was created automatically by mail delivery software (Exim).

A message that you sent could not be delivered to one or more of its
recipients. This is a permanent error. The following address(es) failed:
admin-ccsds-admin at mailman.ccsds.org
    Child process of list_admin_transport transport returned 6 from
command:/var/lib/mailman/mail/wrapper
END OF ERROR MESSAGE:

Also I'm aware that Exim is my MTA, but I don't know what version.  How
can I find out?

I have a funny feeling that the pointer to the wrapper call
(MAILMAN_WRAP) in the exim.conf may be wrong:

EXIM.CONF ENTRY:

# home dir for mailman
MAILMAN0=/var/lib/mailman
# wrapper script for mailman
MAILMAN_WRAP=MAILMAN_HOME/mail/wrapper

I have noticed that in various documentation that the
MAILMAN_WRAP=MAILMAN_HOME/mail/mailman.  Should I change it to mailman?
And if so how do I change it?

All,

I support a Mailman Server (2.1.4) that has over 40 maillists on it.  We
took over support from it in February (Everything is working
great...except) and ever since we have had issues with Admin's not
receiving "Waiting for Moderator Approval E-mails".  Not one time has
any Admin received these messages, so the lists are getting out of
control because the Admins are not moderating.  All of the lists have
"Moderation Emails" enabled, and the old contractor that supported the
site was able to provide these e-mails, because I have example e-mails
form some of the admins.

The caveat here is that when we took over the system we were only given
a backup on the previous system, and we had to start over from scratch.
At the time we decided to take the backup and apply it to a newest
version of mailman at that time (2.1.4).  I'm pretty sure that the
previous contractor used a 2.0.X version, so I wondering if something on
my end need to be changed to enable this.

Every time (that requires or will require moderator approval) I send a
message off to a list I get the follwing errors my log files (filename
-error):
Mail.log - Jun 10 09:05:35 mailman Mailman mail-wrapper: Illegal
command: mailowner
syslog - Jun 10 09:05:35 mailman Mailman mail-wrapper: Illegal command:

Also here is a common example of my alias list entries.  Could it be
possible that the wrapper to mailman change in 2.1 affected the e-mails?

I have followed the guide below.  It was helpful.  When I post a
non-member message to the list that requires moderation I get the
following errors.  Could this be the reason why the admin is not getting
a notification e-mail?

	Well, if the feature is enabled, the next thing I'd look at is 
the Mailman logs of outgoing messages as compared to the MTA logs, as 
well as the cron jobs.  You've got a slightly different issue, but 
the troubleshooting guidelines at

would be a good place to start.
