[Mailman-Users] Subscribed but not subscribed

Hello,

We are running version 2.1.5 on RH Linux 7.3.

There is a list where the "subscribe" log shows a "pending" and "new ... 
via web confirmation" entry for a certain address.  There is no 
"deleted" entry for the address.  Yet the address does not show up on 
the subscriber list.  The address is valid - there were no bounces.

Has anyone else see this behavior?

Is there some way for a deletion to happen without it's being logged?
