I've implemented this (list of addresses and regexps exempt from
approval when subscription requires approval) as a patch to the
2.1.8a1 base. The patch is attached to the above tracker item.

I'm interested in feedback. In order to get it, I'm willing to help
with applying the patch to other recent versions.
