Failure to remove the toolbar is a known issue. See FAQ 3.16:

In Outlook 2003, the "Junk E-mail" folder is a special folder created
by Outlook. Outlook handles it similarly to how it handles the
"Deleted Items" folder, and does not allow it to be deleted. This is
not specific to SpamBayes.
