Summary: doesn't always call endDTD

Initial Comment:
when property_lexical_handler is on, xmlproc calls
startDTD and endDTD on the following doctype
definition:

<!DOCTYPE diagrams-def SYSTEM "diadefs.dtd" []>

but not on those ones:

<!DOCTYPE diagrams-def >
<!DOCTYPE diagrams-def SYSTEM "diadefs.dtd">

where it only calls startDTD
