The easiest way to do this is to do some simple shell scripting
on the server. List all the existing subscriptions, modify them as
appropriate, then delete the old ones and add the new ones.

I'm not personally aware of anyone that has done anything like
this, but I would not be at all surprised if this wheel has been
re-invented several times. You should try looking at the patches on
SourceForge to see if anyone else has uploaded some utilities to do
this sort of stuff.
